What is a TENS device?
A TENS (Transcutaneous Electrical Nerve Stimulation) device is a small, battery-powered remote control that connects to pads or electrodes with wires. Electrodes are placed on specific parts of the body, and the TENS unit delivers an electric current to those places.
TENS machines use soothing electrical impulses that are sent through the skin and along nerve fibers via electrode pads.
These impulses suppress pain signals to the brain, stimulating the body to produce higher levels of its own natural pain chemicals.
You can read more about itRole the brain plays in sciatica HERE.
Many people ask me if TENS can be used to relieve sciatic pain.
For most people, the answer is yes. TENS can relieve sciatica, even the severe pain my clients often experience. TENS machines offer a safe, non-addictive pain relief alternative that can help you relieve discomfort and gain some mobility during a particularly bad sciatica attack.
The best thing about TENS machines for sciatica is that they are very convenient; You can use a TENS unit at home, at work or when travelling. They can be more effective than the traditional pain relief methods that people use during a bad week with their sciatica, like prescription pain relievers, without being addictive and with far LESS side effects. However, TENS devices do not work for everyone and the effect can be very individual.
The science behind TENS units for sciatica pain relief
How exactly can TENS relieve sciatica?
TENS machines send a low-voltage electrical pulse to the skin, which travels to the muscles and nerves. This pulse does three things:
- First, it stimulates your nerves and triggers the production of endorphins, your body's natural painkillers.
- Second, it can also confuse or block pain signals from entering the nervous system and spinal cord.
- Third, the electrical impulses can help relax the contracted muscles in the painful area and provide relief.

What do I need to know about using TENS units for sciatica?
Depending on the level of pain and the desired result, some people can reap great benefits from their TENS unit in as little as a 30 minute session.
Others may need to use the device for longer periods of time, up to several hours a day. But don't worry, it's easy to incorporate using your TENS unit into your daily routine. They fit in your pocket and can go unnoticed by those around you.
When using a TENS machine, you must place at least two pads on your body. This is done to allow electrical current to flow from one pad to the next through skin, nerves and muscles. If you only use one pad, there will be no effect.

Are there any side effects of using TENS machines for sciatica?
In general, TENS machines are very safe and do not have the risk profile of many other treatments or medications.
The side effects that do exist are usually no worse than skin irritation if the electrodes are left in place for several days.
However, patients with pre-existing heart conditions, pacemaker users, and pregnant women should always consult their doctor before beginning the use of a TENS unit.
Research behind TENS devices for sciatica
Although research on the effectiveness of TENS unit therapy for sciatica is somewhat conflicting, most medical professionals agree that the outcome is generally positive.
Approximately 70% to 80% of people with sciatica experience pain relief on their FIRST use of a TENS unit.
However, the main benefit of using a TENS unit is that it offers a window of sorts for people who want to start a sciatica exercise program but are currently limited by severe pain. If the initial pain relief through the use of a TENS device allows you to start gentle exercises, TENS therapy can lead to lasting pain relief!
If you would like a sample sciatica exercise program, here are some suggestions.
When it comes to effectively setting up your TENS unit, research has shown that each level of stimulation provides some level of pain relief.
However, longer lasting and deeper relief occurs when the TENS unit is carefully calibrated to find the right dose. This means you may need to play around a bit to find exactly what setting, duration, and pad placement works best for you. Don't give up too soon if your TENS device doesn't work the first time!
To begin TENS therapy, simply attach the electrodes to the device and then attach the device to the skin on your legs or lower back. Adjust the device until you feel pain relief. As I mentioned before, it can take several attempts to find the right frequency, duration and strength of the electric current.
Placing the electrodes on certain specific locations on the lower back and leg can provide better pain relief.
This video below provides a great explanation of electrode placement:
Placement of TENS pads when using a TENS unit for sciatica
The placement of the TENS pad for sciatica iscriticalwhen using this mode. Getting this right is the most effective way to set up the TENS unit for sciatica and relief.
Where should TENS pads be placed for sciatica and back pain?
One option is to place the electrodes on your lower back near the area of back pain. Adjust the frequency as needed and move the electrodes as needed to get the best pain relief.
The images below should give you an idea of where to best place your cushions.

You should feel relief from TENS very quickly once you find the pad configuration and placement that works best for you.
If you ONLY have sciatica or piriformis syndrome
To relieve leg pain, place the electrodes on the back of the legs. The following images should help with pillow placement.

Adjust the frequency again and move the pads until the pain radiating down the legs subsides.
General advice
I recommend for most people to place one pad near the "top" of the sore area and the other near the "bottom" of the sore area. Most machines allow you to attach up to four pads; A good option is to try two back pillows and two leg pillows.
As I mentioned earlier, try placing the pads on top and bottom of the sore area so that the current flows through the sore area between the pads.
If you find a particularly painful area on your back or legs, it's a good idea to place an electrode there for the most impact.
